You are in your second trimester. You are experiencing sharp type pain or a jabbing type of pain In your lower abdomen and the groin area ( where your legs attach to your pelvic ). As your baby begins to grow in your uterus, these round ligaments stretch. This will cause the ligaments to tighten, something like a rubber band they snap when they are pulled. This will cause you to have a sudden and quick jabbing type sensation. You inform your provider or mid wife and they tell you, you are having round ligaments pains. Do you know what can actually trigger these pains? From below pick the one that is incorrect!

1-Exercises

3- Laughing

4- Rolling over or movement in bed

5- Standing up too quickly 

6- Sitting too long in one position

Its HOT in here!

As the weather changes from cold to mild to hot- One may wonder how do I dress my newborn.  Can I use a fan with the baby in the room.  What is the easiest way to tell if my baby is to hot or too cold.  How do I  tell if my baby is too hot?  Your baby may have flushed cheeks. Feel the nape of the neck to see if it feels sweaty or too cold to the touch.  Over heated babies may have sweating and fast breathing. Which statements below are untrue?

·         When dressing a baby the general rule – dress the baby in as many layer as you have on and then add one more layer.

·         Never ever use a fan in the room with a newborn it can cause pneumonia.

·         Babies that sleep in a less ventilated room sleeping with a fan – lower the risk of Sudden Infant Death Syndrome.

Leaving the ceiling fan on a low setting is not going to cause a cold. Baby should be at least swaddled or in a sleeping sack , socks or booties. Most of the newborn heat escapes from the head as well as their tiny little feet.

Make sure your infant is always dry because damp clothing can cause hypothermia.

Use good parenting judgment-  too hot or not too hot?  Consider the temperature outside.

Should you give you baby a pacifier? Nurse Jackies Tips for Feb 2021

New mothers often ask and wonder if they should give their baby a pacifier?  Do you know the answer? Two answers are incorrect!

  • if you are taking a baby on the plane.
  • If the baby is fussy and has a hard time getting to sleep.
  • If a baby rather uses his thumb or his hand to satisfy himself, you should give the pacifier instead!
  • A pacifier should never be used.
  • Introduce a pacifier approximately 3-4 weeks after breastfeeding has been established.

You are going to a small intimate affair (keeping in compliance with Covid 19 guidelines)  Wait!! I’m pregnant-can I dance?

Dancing is a safe and fun way to exercise during pregnancy.   If dancing is new to you, it’s best to stick to a dance that doesn’t involve jumping and isn’t too strenuous. Consult with your physician or midwife that you’re dancing and exercising during pregnancy!   Not only do you get to move your body to music you love, but it will keep you flexible while toning your muscles. You can get a cardiovascular workout from any fast paced dance, or stretch and maintain muscle tone when you hold positions in yoga or ballet.  For the average pregnant woman, dancing and aerobic activity is safe during pregnancy.  Pregnancy is not a disease and women should feel comfortable being active. If something hurts, don’t do  it!!!!!

If I dance can it cause a miscarriage?

The pregnancy separates from the uterus and passes out of the body. Miscarriage is not caused by the activities of a healthy pregnant woman, such as jumping, vigorous exercise and vaginal intercourse.  However, it is recommended to be gentle.  Trauma causes miscarriage only very rarely unless the impact to the abdomen occurs >12 weeks of pregnancy when the uterus rises out of the pelvis. Stress and emotional shock do not cause miscarriage either.  When you are doing movements you should try to keep both feet on the floor because it is easier for a pregnant person to lose her balance because the growing abdomen shifts the center of gravity forward.  If you do fall by chance you or hit your abdomen, you  should go to the hospital immediately and contact your physician or midwife. Always check with your provider if dancing and or exercises is permitted for you . Each and every  pregnancy is different and each and every person’s body is different. What’s good for one person may not be good for another person!  Be safe and have fun. Happy New Years !!

It’s hard to be pregnant during the holiday season .

We’re preparing, baking cookies, going to holiday parties, shopping in a frenzy for that perfect  gift. About to just celebrate the season with friendship, great food and drinks.  This can be the most wonderful time of the year. But when pregnant or trying to get pregnant, it can be challenging to face the decision about whether to celebrate the season with or without alcohol.

Can I have just a little something?

According to the Centers for Disease Control and Prevention, the American College of Obstetricians and Gynecology, and the American Academy of Pediatricians, “there is no safe amount of alcohol, no safe type of alcohol and no safe time to drink when a woman is pregnant.” And yet, in 2016, the CDC estimated that 3.3 million women in the United States were at risk for having an alcohol-exposed pregnancy.

When a woman drinks so does the developing baby, who lacks the ability to process or metabolize alcohol through their liver or other organs. The baby has the same blood alcohol concentration as the mother. It makes no difference if the alcoholic drink consumed is a beer, glass of wine or a distilled spirit or liquor such as vodka.

What are the risks of alcohol and pregnancy?

Evidence-based research has found that drinking even small amounts of alcohol while pregnant can increase the risk of miscarriage, stillbirth, prematurity or sudden infant death syndrome. In addition, more than 40 years of published research has shown alcohol to be a neurotoxin in utero. This means that alcohol is a toxic substance to the developing baby just like carbon monoxide and lead.

Alcohol can cause the death of developing brain cells, even during the early stages of pregnancy. Most babies negatively affected by alcohol exposure have no apparent physical birth defects. But they can have lifelong behavioral and learning problems that often go undiagnosed or misdiagnosed as autism or attention deficit disorder instead of one of the fetal alcohol spectrum disorders (FASDs). Current studies estimate that 1 in 20 school-age children may have FASDs.

There is no known safe amount of alcohol use during pregnancy or while trying to get pregnant. There is also no safe time during pregnancy to drink. All types of alcohol are equally harmful, including all wines and beer.  There is also no safe amount of drugs including marijuana that we know of.  Not consuming alcohol at any time during pregnancy is the current recommendation, you can always order mocktails, nonalcoholic beverages or sparkling water with fresh fruit. They can be very refreshing and festive as well as enjoyable and help you get your holiday spirits on during this holiday season and throughout your pregnancy.

You are full-term and it is your first child -How should you proceed with labor?

You are full-term and  it is  your first child.  The baby is moving just fine . You are having  contractions that are strong and yet tolerable. Your Birthing Plan is too do this naturally.

How should you proceed with this labor? 

  • Go to your  facility or hospital immediately  to be checked 
  • First baby may take a long time , walk, use a birthing ball, rock/ dance, Breathing  and relaxation exercises, shower  or tub ( tub if not GBS positive and did not break your water bag)
  • Check your bag  for needed item such as insurance card  , ID,  etc.
  • Make sure you eat a hearty meal because your facility or hospital may not allow you to eat    You don’t want to starve !

You are breastfeeding and you find your nipples to be a little cracked ! Nurse Jackies Tip for 7-6-20

What can cause cracked nipples during breast feeding?

Choose the incorrect answer.

  • The baby is not latching on properly
  • You need to reevaluate your sitting position when breast feeding 
  • You are using soap  to wash your breast and that can cause skin cracks in the nipple 
  • It is unknown why the nipples crack,  but it does occur.  This is normal and it is expected to happen.  There is no need to intervene. The nipple will heal!

Should baby be placed on stomach or chest after birth? Nurse Jackies Tips for 6/1/20

You are finally having your bundle of joy! You have read and your delivery assistant has told you – that when the baby is born it should immediately be placed on your chest for bonding, but your provider places the baby on your stomach and not your chest – Do you know why? Which 2 are the correct answers?

  1. Your provider does not agree with bonding on the chest.
  1. Easier for the provider if the baby is in the abdomen as oppose to  the chest.

3.  Your baby may have a short cord and the cord should not be pulled on otherwise it can pull off and become an emergency

4. Your provider might be focused on something happening vaginally or bleeding from the uterus and needs to pay attention to that – so it was easier to place the baby on the abdomen first.

Babies are born clean!

Vernix – what is it and advantages and disadvantages of wiping off…

When a baby is born it should be vigorous – this means the baby breathing, heartrate is over 100, color is pink, crying and able to move all limbs .  If this is not occurring the baby will need to be suctioned, given oxygen, dried , warmed up , stimulated. Drying and stimulating is done with effort to stimulate the heart, promote circulation and provide warmth. In providing this care vernix may be almost or completely removed as part of the process – not purposely but in getting the fluids off because drying will help to keep baby warm.  The vernix on the baby at this time is  not important then the baby is unresponsive, getting the baby to a living state is. If the baby is vigorous then by all means do not have to remove the vernix. Baby safety is everything!!!  Common sense is important.  Immediate prompt and proper care of this baby is essential! If it is favorable leaving the vernix on the baby is beneficial . 

Vernix caseosa, also known as vernix, is the waxy or cheese-like white substance found coating the skin of newborn human babies. It is produced by dead skin cells and is thought to have some protective roles during fetal development and for a few hours after birth.

Vernix: Don’t Wash It Off, Rub It In

Most doctors and nurses want to wipe vernix off baby immediately after birth, but vernix offers many potential benefits for baby when you leave it on.

You’ve just given birth to your beautiful little baby and you’re all ready for cuddles. You’ve been waiting for 9 months for this moment!  But wait… What’s that white, waxy, cheesy substance coating baby’s skin? Yeesh, that’s kinda gross.

That waxy stuff is called vernix

And yes, the sticky, white, cheese-like coating covering your precious little one may not look that appealing. Which is why birth center or hospital staff may want to wash baby off right away.

Ask them not to wipe it off if all has gone well and your baby is pink, perky and active!  Vernix offers a number of benefits for baby when you leave it on or rub it in.  The best time to have that discussion is on admission – during labor.  Speak to nurse and ob provider so you are all on the same page.

What is vernix?

Vernix begins to form on the unborn baby at about 20 weeks gestation, partially to prevent baby’s skin from getting too waterlogged after marinating in amniotic fluid month after month. Unlike skin cells, vernix is more mobile and fluid. It is also more permeable to the transport of water and other small molecules to the baby. (source)

Even though it helps protect baby’s skin from amniotic fluid, the creamy vernix itself contains about 80% water. There are plenty of beneficial components to the vernix. Scientists have identified lipids, amino acids, proteins, antibacterial, and antimicrobial compounds including:

  • wax and sterol esters
  • ceramides
  • squalene
  • cholesterol
  • triglycerides
  • phospholipids
  • and amino acids, asparagine and glutamine

About 61% of the proteins found in this white substance, can only be found in vernix. And humans are the only ones that produce it, making it truly unique. 

Vernix begins to form on the unborn baby at about 27 weeks gestation, in the third trimester.
